Well, the idea behind this, (and why the midwife is in a neighbouring village), is that she's the nearest midwife to them. (In fact, the only sim I have at the moment who I could see being a midwife is Bethany, and even then I'm not sure she's strong enough mentally and emotionally for it). Plus I don't think it was an easy thing to do, so she's quite hard because she has to be, and is jaded to boot.
